In some circumstances, the information obtained from a social determinants of health (SDOH) assessment tool not only improves the immediate health care provided, but also creates an opportunity for a patient to be referred for other services unrelated to the visit that will impact other areas of the patient’s health. Once an assessment identifies a social need, clinicians or health care providers must refer patients to organizations for additional resources and services. To ensure referrals to high-quality external resources, health care providers should consult trusted clearinghouses of quality providers such as those listed in this document.
